Mechanism of Action

Norvasc is primarily indicated for the treatment of hypertension, dyslipidemia, and angina. There are no adequate and well-controlled trials in patients with heart failure or hypertension who were taking any of the following medications at a low dose: ACE inhibitors (e.g., benazepril, otherenin), beta-blockers (e.g., doxazosin, tamsulosin), alpha-blockers (e.g., prazosin, terazosin), or calcium channel blockers (e.g., amlodipine, atenolol, doxazosin). The data suggest that it is generally not beneficial to exceed 200 mg of Norvasc per day or calcium-channel blocker, or potassium-sparing diuretics (e.g., spironolactone). However, a single daily dose of 50 mg of Norvasc may be sufficient to reduce blood pressure to 100 to 110/50 mm Hg and improve general organ function in heart failure patients. A single daily dose of calcium-channel blockers or potassium-sparing diuretics (e.g., spironolact or tamsulosin) may be sufficient to control blood pressure in patients with heart failure or hypertension. In patients with heart failure or hypertension, the dose should be divided into multiple doses to control blood pressure more effectively. In addition, the elderly have renal impairment and should be monitored for signs and symptoms of hypokalemia, including hyperkalemia, and hyponatremia. Hyperkalemia and hyponatremia have been reported in association with Norvasc administration and can occur at any time during therapy. Hyperkalemia has been suggested as a possible symptom of diabetes mellitus and may occur as a consequence of Norvasc or other diabetes medications. Hyponatremia has been reported in association with the concomitant administration of Norvasc with antihypertensive agents. A single dose of calcium-channel blockers or potassium-sparing diuretics (e.g., spironolact or tamsulosin) may be sufficient to control blood pressure in patients with heart failure. It may be determined by body weight in patients with heart failure or hypertension if the necessary dose is required. The data suggest that it is not beneficial to exceed 200 mg of Norvasc per day or calcium-channel blocker, or potassium-sparing diuretics (e.g., spironolact or tamsulosin) and calcium-channel blockers (e.g., amlodipine, atenolol, doxazosin), potassium-sparing diuretics (e.g., spironolact), or potassium-sparing diuretics with calcium-channel blockers (e.g., amlodipine, tamsulosin) per day or calcium-channel blockers (e.g., potassium-sparing diuretics) daily. A single daily dose of Norvasc 50 mg may be sufficient to reduce blood pressure to 100 to 110/50 mm Hg and improve the organ function in heart failure patients.
